Care Plan Summary Ms Walters sustained a hip fracture and a fractured shoulder 6 months ago following a road traffic accident and has not made a full recovery. She is having difficulties with mobilising and carrying out personal care and dressing tasks. Personal care is being provided and rehab services to help Ms Walters to improve ADLs and mobility, with a review planned in 8 weeks.
